Date of birth The date of birth is questionable. She kept telling me it was 2/16 but then saying no, it was 12/16. She then told me that the year of birth was 1997. At the next session, I will ask a teacher as to the correct birthdate and year.

Describe the mood of the child when you began to work with him or her:

She appeared to be in a very good, happy mood. She was smiling when I first met her and was quiet but also talkative and friendly.

Please describe the physical conditions that apply to your child today.

Today, she had no physical problems or conditions. She was in good health.

How did you begin this conversation?

A teacher brought her up to the room where I was while I was finishing with the other child and asked me if I need her (2013). I then told her my name, asked her how she was doing and told her what we would be doing.

What was your opening line?

My opening line was "Hi, My name's Liz. How are you?...I'm going to be talking to you for a project I'm working on. First of all, before we start anything, I would like to take a picture of you. Is that okay?...Anywhere you want. Just picke a spot that you would like it to be in."

What were the topics you covered?

The majority of our conversation dealt with school and what she liked about it, how she got there, what she did...We also talked about her outfit, my outfit and her family.

What topics seemed most interesting to the child?

She liked telling me about her Kindergarten and what she did there. She also liked to talk about her family.

Who led the conversation?

I led the conversation because she seemed a little shy at first. Whenever I initiated a topic though, she was very eager to respond and tell me what she thought. As the conversation went on, she opened up more and got more excited about it.

Were there continuing themes?

Yes. She told me a lot about her school. She was very proud of it and of the fact that she was in Kindergarten.

Did the child play with any toys or do any art activities (instigated by the child)

Yes. We worked on coloring a picture together and decorating the first page of her book while we talked. I think it helped her get more comfortable with me by working on the coloring. She said that she wanted to make a picture together and that's what we did.
